Transition and School Support

Transition Services

Leaving high school is a big life change for graduates and their families. Planning for the future allows students and families to think about ways to help the student achieve his or her dreams and life goals, as well as make connections to the post-graduation services they need. Transition also includes experiences focusing on community employment.

Starting at age 14, transition consultants from Hamilton County DD Services will meet with families and students to help ease the student's transition from school to adulthood. Transition services are available for any student who receives services from the agency.

Itinerant Support

The Hamilton County DD Services Itinerant Support Team collaborates with school-based teams to support children with developmental disabilities and moderate to intensive needs in their community schools. 

Services provided include some or all of the following: observations, special team meeting participation, strength-based curriculum recommendations, classroom organization and structural support, visual supports training, crisis prevention and management support, proactive vs. reactive teaching recommendations, reinforcement and motivation training, specialized trainings as needed, and additional support from occupational, speech and physical therapists as needed.

To receive services, the student must be a resident of Hamilton County with an active Individulized Education Program (IEP) and Evaluation Team Report (ETR). Referral forms must be completed with assistance from the child's guardian and support team.

Behavior Support

The purpose of behavior support is to help students to exercise choice in ways that foster positive academic and functional outcomes by promoting positive relationships that are achieved through supportive action. The student must feel safe, cared about and be actively engaged with the people in his/her life for these relational supports to be effective. HCDDS schools seek to develop such meaningful relationships in the lives of students.

Agency staff believe the school environment should be one that ensures the care, safety, and welfare of all students and staff members. The Behavior Support Team advocates for the use of positive support strategies, choice making, and independence across all environments and circumstances.
